Contents Piperacillin Na, tazobactam Na Indications / Uses Adult & adolescents: Treatment of moderate to severe infections in the lower resp tract including severe community-acquired & healthcare-associated pneumonia; uncomplicated & complicated skin & skin structure infections including cellulitis, cutaneous abscesses & acute ischemic/diabetic foot infections; intra-abdominal infections including appendicitis (complicated by rupture & abscess) & peritonitis; complicated & uncomplicated UTI; gynecologic infection including postpartum endometritis or pelvic inflammatory disease; bone & joint infections; bacterial sepsis; bacterial infections in neutropenic patients. Polymicrobic infections including those where aerobic & anaerobic organisms are suspected (intra-abdominal, skin & skin structure, upper & lower resp tract, gynecological). Childn: Complicated intra-abdominal infections in childn ≥2 mth & bacterial infections in febrile neutropenic childn 2-12 yr.

Dosage / Direction for Use Adult Most infections 4 g/500 mg every 8 hr. Total daily dose: 2 g/250 mg to 4 g/500 mg every 6, 8 or 12 hr for 5-14 days. Healthcare-associated pneumonia 4 g/500 mg w/ aminoglycoside every 6 hr for 7-14 days. Bacterial infections in neutropenic patient 4 g/500 mg w/ aminoglycoside every 8 hr. Total daily dose: 2 g/250 mg to 4 g/500 mg every 6 or 8 hr. Hospitalized childn (≥2 mth) w/ complicated intra-abdominal infections: ≥9 mth or weighing >40 kg Follow adult dose, ≥9 mth or weighing ≤40 kg 100 mg/12.5 mg/kg/dose every 8 hr, 2-9 mth 80 mg/kg/dose every 8 hr. Neutropenic childn (2-12 yr) w/ fever suspected to be due to bacterial infections 80 mg/100 mg/kg/dose every 6 hr. Contraindications Hypersensitivity to penicillins, cephalosporins & β-lactamase inhibitors. Special Precautions Serious anaphylactic/anaphylactoid reaction. Clostridium-difficile-associated diarrhea. Patients w/ bleeding manifestations, convulsion, restricted salt intake, hypokalemia, CrCl of ≤40 mL/min. Perform periodic assessment of organ system function including renal, hepatic & hematopoietic. Not recommended in the treatment of meningitis & brain abscess. Consider severity of infection & the prevalence of resistance to other suitable antibacterial. Increases risk of the development of drug-resistant microorganisms. Long-term or repeated use may result in overgrowth of nonsusceptible organisms including fungi. Pregnancy & lactation. Elderly. Childn ≤2 mth. Side Effects / Adverse Reactions Allergic reactions, erythema, pruritus, rash (including maculopapular, bullous, urticarial & eczematoid), urticaria, dyspepsia, diarrhea, nausea, vomiting, superinfection, phlebitis & insomnia. Interactions Substantial inactivation of aminoglycosides. May affect the blood coagulation system &/or the thrombocyte function w/ heparin, oral anticoagulants & other drugs affecting the blood. Methotrexate toxicity. Prolonged neuromuscular blockade w/ vecuronium. Longer t½ & renal clearance w/ probenecid.
